1. Open the Informed Consent entry
Entry path: Form editor → Survey → Informed Consent
- Open the Smart Form you want to edit and enter the form editing interface.
- In the left navigation area, select the Survey module.
- Locate the Informed Consent option.
- Click Informed Consent to open the related editing area.
Success Criteria
- Interface result: The informed consent editing view or settings panel opens.
- Next action available: You can continue adjusting settings or reviewing the current configuration.
Tips While Using
- Make sure the current form is in an editable state.
- If you cannot find the entry, first confirm that you are inside the Survey module.
2. Complete the Informed Consent setup

- In the Informed Consent settings panel, adjust the related options as prompted in the interface.
- Use preview to check the display result after the setup is complete.
- After confirming the content is correct, click OK to finish.
Success Criteria
- Interface result: The configured result displays correctly in preview.
- Next action available: You can return to the form, continue editing, save, or publish.
Tips While Using
- It is a good idea to preview the informed consent content once before publishing.
